Position Description

Summary: The Registered Dietician/Nutritionist supports the primary care providers by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression, complications, cognitive and functional decline. In collaboration with the provider and any interdisciplinary team, the Registered Dietitian develops and implements an individualized care plan designed to promote the patients understanding and management of their condition, optimize quality of life, and improve clinical outcomes. This role will also develop various nutritional programs for the entire organization and will provide diabetic education through the accredited Diabetes Self-Management Education and Support (DSMES) program. This position provides service in the ambulatory care setting at all MLMC clinical sites.

Essential Duties and Responsibilities:

Diabetes Education and Disease Prevention

Provides education to patients within the curriculum of MLMCs DSMES program.

Accurately documents patient progress through the DSMES program and is responsible for continued certification of the program as the DSMES Quality Coordinator.

Coordinate with nursing staff for the insertion, monitoring, and removal of continuous glucose monitors (CGM) if needed. Coordinate with provider for follow-up counseling with the patient.

Under the direction of medical provider, assists patients with diabetic medication adjustments.

Stays up to date with evidence-based practice regarding diabetes and serves as a resource to MLMC staff and patients.

Promotes the DSMES program within MLMC and throughout the community to maintain program participation volumes.

Monitors and updates the DSMES curriculum as needed.

Participate in internal and external committees/community education related to diabetic care.



Dietary Evaluation and Education

Conduct comprehensive nutritional assessment, education, and management to patients/clients referred by providers and/or multidisciplinary team.

Provide instruction to patients in the use of modified diets as it relates to health promotion and chronic conditions.

Coordinate, implement, and evaluate nutritional education using National standards and guidelines.

Evaluate patients for needed services and provide the medical treatment team with assessments and recommendations.

Develop and deliver educational programs and appropriate teaching tools for both medical treatment team and patients to support understanding of nutrition.

Provide outpatient and community-based education and counseling to patients and families, individual or group, in accordance with established educational plans.

Maintain timely and complete documentation and billing of all care management encounters.



Patient Care

Provides direct service to patients within the approved scope of licensure.

Monitors and ensures accuracy of recording on patient's electronic medical records; documents all activities as required by professional licensure; maintains patients electronic files.

Participates in the research and development of assistance programs for uninsured patients.

Monitors medical conditions of patients and reports changes to appropriate clinical personnel.

Maintains a safe, comfortable, and therapeutic environment for patients/families.

Inspects area daily for compliance with policies, procedures, regulations, and standards; ensures an adequate stock of medical supplies and proper functioning of equipment.

Provides excellent patient centered customer service while being friendly and courteous to all employees and patients; can handle multiple tasks at once and has the ability to compassionately and empathetically care for patients.

Job Qualifications

Qualifications:

Bachelor's degree in Nutrition Science from a program accredited by the Commission on Accreditation for Dietetics Education

Completed all necessary supervised internship work.

Passed a national examination administered by the Commission on Dietetic Registration.

Be eligible for licensure as a registered dietician in Maryland.

Knowledge of nutrition guidelines in chronic disease management.

Knowledge of medical terminology.

Knowledge of HEDIS or NCQA quality measures



Understanding of HealthCare laws and Regulations:

Must have the ability to follow HIPAA guidelines as well as an understanding of policies and procedures regarding medical records put in place by the Federal Government.



Computer Literacy:
Extensive knowledge of computer software and proficiency in or knowledge of Microsoft Office programs such as: Excel and Microsoft Word. Experience working with an Electronic Health Record.



Reasoning Ability:

Must be able to demonstrate the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.



Teamwork:

An essential element is the ability to work and interact effectively and positively with other staff members to build and enhance teamwork in a patient centered team-based model of care and in the overall MLMC organization.



Work Record:

A demonstrated work record showing good attendance, punctuality, dependability and the ability to work well with supervisors and coworkers is essential. A medical office setting is an environment that requires the ability to relate to all types of people while always maintaining a professional demeanor.



Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals is essential. Ability to write routine reports and correspondence, and the ability to speak effectively to patients, co-workers and the public at large is paramount.



Sensitivity to Needs of Special Populations

Must be able to demonstrate an understanding and sensitivity to cultural differences and needs of MLMC patients and be able to respond appropriately to all people regardless of race, ethnicity, language, age, sex or sexual orientation, religion, disability, economic standing, etc.



Physical Demands

The work environment is a typical outpatient medical clinic. While performing the duties of this job, candidates may be required to drive, sit for extended periods, spend time standing and walking, handle considerable paperwork, operate computers, lift up to 15-pound boxes of supplies and assist patients who may have limited mobility.



Work Environment

Work is performed in a typical, outpatient clinic environment and possibly at locations outside the office. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job.
